2027 Maximum Broker Commissions for Medicare Advantage & Medicare Part D

What are agent commissions for Medicare Advantage plans in 2027? What about Part D plan commissions? We’ve got your answers.

For 2027, we see an increase in commissions, especially for Medicare Part D plans.

Medicare Part D Commissions

How are agents compensated under CMS rules for Medicare Part D? For 2027 Part D commissions…

  • Initial commissions will increase from $114/member/year to $130/member/year, a 14% increase YOY.

  • Renewal commissions will increase from $57/member/year to $65/member/year, a 14% increase YOY.

Medicare Advantage Commissions

The 2027 Medicare Advantage commissions are broken out by state/region:

  • For CT, PA and DC, initial Medicare Advantage commissions will increase from $781/member/year to $816/member/year, a 4.5% change YOY. Renewal commissions will increase from $391/member/year to $408/member/year, a 4.3% change YOY.

  • For CA and NJ, 2027 initial Medicare Advantage commissions will increase from $864/member/year to $902/member/year, a 4.4% change YOY. 2027 renewal Medicare Advantage commissions will increase from $432/member/year to $451/member/year, a 4.4% change YOY.

  • For Puerto Rico and the U.S. Virgin Islands, initial MA commissions will increase from $474/member/year to $495/member/year, a 4.4% change YOY. Renewal commissions increase from $237/member/year to $248/member/year, a 4.6% change YOY.

  • In all other states, initial MA commissions will increase from $694/member/year to $725/member/year, a 4.5% change YOY. Renewal commissions will increase from $347/member/year to $363/member/year, a 4.6% change YOY.

Compensation Rate Submission for 2027

It’s important to note, CMS expects organizations to keep full records documenting that they are updating compensation schedules and paying agents and brokers according to CMS requirements.

Additionally, to enhance transparency in agent and broker compensation, CMS is requesting that Medicare Advantage (MA) and Part D plans voluntarily submit additional compensation data beyond the existing reporting requirements. These voluntary data fields will remain open and may be updated at any point throughout the plan year.

Compensation rate submission for 2027 requires organizations to report to CMS if they intend to use employed, captive, or independent agents or brokers in the upcoming plan year along with the specific rates the plan will pay independent agents and brokers.

In addition, if an organization pays referral fees, they must disclose the amount. For 2027, referral fees remain the same at $100 for MA plans and $25 for PDP plans.

MA Broker Training and Testing

Agents are expected to possess a high level of knowledge, which is achieved through the required annual Medicare training such as AHIP or NABIP.
