Elevance to Require Paper Apps for 2025 MAPD PPO Plans in Most States

An important announcement from Elevance: Elevance has decided to make their Medicare Advantage Prescription Drug (MAPD) PPO plans non-marketable for 2025 in all states except Ohio.

As a result, in all states other than Ohio, brokers will be unable to submit Elevance MAPD PPO enrollments electronically through online platforms, including SunFire, Connecture, MedicareCENTER, and Elevance’s platform.

Writing Elevance 2025 MAPD PPO Applications

All Elevance plans in all states will remain commissionable, but brokers in all affected states will need to submit 2025 MAPD PPO enrollments by faxing the paper application to Elevance. This applies to all brands under the Elevance umbrella that offer these plans, including Anthem and Simply Healthcare Plans.

Will Elevance Accept Online Enrollments for MA Plans?

MA-only PPO plans and D-SNP PPO plans, as well as MAPD HMO plans, are still marketable to quote and will remain available on platforms for online enrollment, including MedicareCENTER or your preferred enrollment platform. Paper enrollments may also be submitted for these plans.

Where to Find Elevance MA Paper Applications & Sales Kits

Brokers can download Anthem paper sales kits here and Simply paper sales kits here. Enter the client’s zip code, navigate to the desired plan, and click on View Plan Documents to view and download all the associated plan information and enrollment application. The fax number to submit the paper application is included on each individual enrollment form.
